Event Medical Assistance Request Form

The Elizabeth Fire Protection District is pleased to offer roving EMS related services for public community events taking place within our District.

Please note that requests for a medical presence at an event must be received by the District no fewer than 30 days before the event. This request form is only for on-site roving EMS services: we cannot provide an ambulance for your event. Services will be provided starting at $100 an hour for no fewer than 2 hours with 2 EMS providers minimum. Please be advised that, although the District will make every effort to accommodate your request, approval is not guaranteed. Please contact the District with any questions or for more information.
